YOUR VISIT
Our goal is to make your office visit as pleasant as possible. To assist with this process, please review this page for useful information to help you prepare for your upcoming appointment.
If this is your first visit to our office, or if you are a returning patient who has not been seen in over a year, please arrive 15 minutes early. Returning patients should inform the receptionist if there are any changes in address, phone number, insurance coverage, marital status, etc.
New and returning patients will need to bring the following:
- Insurance Card(s)
- Photo I.D.
- Co-payment
We understand that your time is valuable and we always do our best to be on time for your appointment. Emergencies do arise which can cause delays and so we ask for your understanding if this occurs.
Parking
ENT Medical Services has ample parking available for your convenience. No need to worry about parking ramps or paying meters when you are seen at our office. Patients can find free parking at the West or East entrances of the building.
PATIENT FORMS
For your convenience, we have included patient forms which are displayed below. If you wish to complete paperwork prior to your visit, please download the applicable form, complete in full, and bring with you to your appointment. If you are unable to do so, we ask that you arrive 15 minutes prior to your scheduled appointment time.
If you are a new patient, or if you are an established patient who has not been seen in over a year, we will require a New Patient Registration Form to be completed in full prior to being seen at our office.
Prescription Policy
Prescriptions and refill requests are granted during regular office hours. When requesting a prescription, please have your pharmacy phone number available or have your pharmacist call us at (319) 351-5680.
