Your Visit

Our goal is to make your office visit as pleasant as possible. If this is your first visit to our office, please arrive 15 minutes early to ensure enough time to complete your registration.

Patient Forms

For your convenience, you can download a new patient registration form to fill out in advance of your visit. Please complete the form in its entirety before coming in for your appointment. If any patient information has changed since your last visit, for example your insurance carrier or your address, please complete another Patient Registration form as well. In addition to the completed registration form, please bring the following documents with you to your appointment:

  • Insurance Card
  • Photo ID
  • Co-Payment
  • Referrals (If required by your insurance company)

Parking

ENT Medical Services has ample onsite parking for your convenience.

Appointments

Office hours are 8:00 AM to 4:30 PM, Monday through Friday. Office appointments are scheduled by calling (319) 351-5680 or (800) 642-6217.

We understand that your time is valuable and we always do our best to be on time for your appointment. Emergencies do arise and cause delays and we hope you will understand if this occurs.

To contact us in an emergency call Mercy Hospital (319) 339-0300 or (800) 637-2942 and ask for the ENT Medical Services doctor on call.

Insurance & Billing

You will be asked to present your insurance card with each visit. Please tell the receptionist if there are any changes in address, phone number, insurance coverage, marital status etc.

Financial Policy

There is a fee for our services. We participate and file claims with the following companies:

  • Medicare
  • Wellmark BCBS
  • Iowa Medicaid
  • UnitedHealthcare of the River Valley
  • Mercy Priority Health Network (with some exceptions)
  • UnitedHealthcare
  • Co-pays are due at the time of service.

Your health insurance is a contract between you and your insurance company. It is your responsibility to know your benefits and pursue unpaid claims.

For your convenience we accept Visa, MasterCard, CareCredit, personal checks, and cash as a means of payment. Monthly payment plans are available and can be arranged by contacting our billing department. Our staff will be happy to answer your questions regarding insurance, account balances and payment plans.

Cosmetic Procedures

Payment is to be paid in full 10 days prior to the procedure.

Prescription Policy

Prescriptions and refill authorizations are granted during regular office hours. Please have your pharmacy phone number available or have your pharmacist call us at (319) 351-5680.
